Moderator: Scott Shreeve, MD - Chief Executive Officer at Crossover Health

Dr. Shreeve is a board-certified Emergency Medicine physician who has been actively involved in the design, development, and distribution of technologies that improve care delivery and enhance quality of life. He is the Founder and Chief Executive Officer of Crossover Health, a next-generation healthcare delivery and management services company fundamentally changing how care is practiced, delivered, and experienced. Dr. Shreeve was a co-founder of Medsphere Systems Corporation, the first open source electronic health record for the healthcare enterprise, where he led large scale electronic health record implementations at community hospitals, state wide health systems, and networks of Primary Care clinics. Dr. Shreeve is also a founding member of the Health 2.0 technology and reform movement, serving as an advisor to the conference and spokesperson throughout the country.









Panelist: Stephen Ezeji-Okoye, MD, Chief Medical Officer at Crossover Health

Stephen Ezeji-Okoye, MD, is Chief Medical Officer (CMO) at Crossover Health. With a passion for population health and redesigning systems to drive improvements in quality and efficiency, he manages the delivery of outcomes-based, effective clinical care at Crossover.

Previously, Dr. Ezeji-Okoye led ambulatory care service and oversaw clinical operations as Deputy Chief of Staff at the VA Palo Alto (Calif.) Health Care System (VAPAHCS). Dr. Ezeji-Okoye also served as Clinical Professor (affiliated) at Stanford University School of Medicine, a national consultant to the VA on the use of integrative medicine practices in VA care, and as an advisory council member to the National Institute of Health’s National Center for Complementary and Integrative Health.

Dr. Ezeji-Okoye graduated from Harvard College and attended medical school at the University of Texas Health Science Center at Houston. He completed his internship, residency, and chief residency in internal medicine at Stanford University.







Panelist: Dena Bravata, MD, MS

Dena Bravata is a healthcare entrepreneur and executive. Dr. Bravata is the co-founder of Lyra Health where she also served as the Chief Medical Officer from 2015 to 2016. She was the founding Chief Medical Officer and Head of Products at Castlight Health from 2008 through 2014 and again from 2019 to 2022 (formerly NYSE:CSLT). Dr. Bravata was a practicing internist for 16 years in her own private practice and an attending physician at Stanford University and the Palo Alto VA.

As a nationally recognized health services researcher, Dr. Bravata has published widely on the clinical effectiveness of a variety of key health technology, clinical, and public health topics. She is a founding member of the Aspen Institute Healthcare Strategy Group, was an assistant public health officer for Santa Clara County, and serves on the board of several health technology and services companies. In 2021, Dr. Bravata received a national service award from the White House and CDC for her work on the pandemic response and Vaccines.gov. She has also been a Crossover advisor since 2015 and co-authored several of Crossover’s peer-reviewed publications, including a systemic review of Imposter Syndrome that is one of the most cited papers ever published in the Journal of General Internal Medicine.

Dr. Bravata has an undergraduate degree from Yale University, an M.D. from Columbia University, and an M.S. in health policy from Stanford University. She completed her Internal Medicine Residency and Ambulatory Care Fellowship at Stanford University.









Panelist: Melissa Hogan, Senior Vice President at Aon

Melissa Hogan is a Senior Vice President within Aon’s Health Transformation Team. In this role, Melissa partners with client leaders and colleagues to support care delivery. She leads Aon’s Advanced Primary Care consulting, employer-sponsored health, and occupational clinic and navigation practices with additional focus on chronic condition management.

She began her career in clinical research in the medical university setting, then moved into the public sector addressing population health, and has been working in benefits and human resources with an emphasis on clinics since 2007. Prior to joining Aon in 2017, Melissa had the role of senior director of the St. Louis Area Business Health Coalition, where she served more than 60 public and private sector employers on their benefit strategies through group purchasing, direct contracting, and population health initiatives.

Melissa has spoken nationally on Primary Care delivery, employer-sponsored clinics, and provider quality improvement initiatives and served on the board of various national, local, and state health and advocacy agencies including the Clinical Program Committee for the National Committee for Quality Assurance.

Melissa earned a Bachelor of Special Studies in communications and psychology from Cornell College. She also has a master’s degree in public health from Saint Louis University. She is a Patient Centered Medical Home Certified Content Expert (PCMH CCE) through the NCQA with 17 years experience developing, implementing, and evaluating care delivery opportunities.
