Heuseveldt is the Health Coach Senior Program Manager at Crossover. She’s a registered dietitian and board-certified health & wellness coach. She earned her Bachelor's Degree in Nutrition from Marywood University and has been working in employee health & wellness since 2007.

Moderator: Nate Murray, Founder and Chief Revenue Officer at Crossover Health

As Chief Revenue Officer and co-Founder of Crossover Health, Nate brings more than 20 years’ experience in and passion for healthcare to his daily interactions with clients. He’s a strong advocate of plan designs that make Primary Care, Mental Health, and Physical Medicine low- to no-cost for members. He has developed shared savings programs that align incentives across partnerships and deliver objective improvements across cost, quality, and experience.

Prior to founding Crossover, Nate led the development of a direct-to-consumer Medicare marketing program and built a large online Medicare broker partner network. He has also worked in Operations and Product Development for several large healthcare payers and has expertise in healthcare technology and helping payers and consumers make cost-effective decisions.

Nate earned his Bachelor of Arts degree in Economics from the University of Utah and holds a Master of Business Administration degree from Brigham Young University.

Panelist: Stephen Ezeji-Okoye, MD, Chief Medical Officer at Crossover Health

Stephen Ezeji-Okoye, MD, is Chief Medical Officer (CMO) at Crossover Health. With a passion for population health and redesigning systems to drive improvements in quality and efficiency, he manages the delivery of outcomes-based, effective clinical care at Crossover.

Previously, Dr. Ezeji-Okoye led ambulatory care service and oversaw clinical operations as Deputy Chief of Staff at the VA Palo Alto (Calif.) Health Care System (VAPAHCS). Dr. Ezeji-Okoye also served as Clinical Professor (affiliated) at Stanford University School of Medicine, a national consultant to the VA on the use of integrative medicine practices in VA care, and as an advisory council member to the National Institute of Health’s National Center for Complementary and Integrative Health.

Dr. Ezeji-Okoye graduated from Harvard College and attended medical school at the University of Texas Health Science Center at Houston. He completed his internship, residency, and chief residency in internal medicine at Stanford University.


Panelist: Michael Tong, ,Pharmacist at A-S Medication Solutions

Michael Tong is a staff pharmacist for A-S Medication Solutions at a dedicated employer pharmacy in Mesquite, Texas. Michael has been a pharmacist for over 23 years and his passion is in advocating for patients and improving their outcomes. Michael believes in the dedicated employer health model and has seen first hand the benefits of an integrated pharmacy solution with a health center. Michael graduated from Rutgers College of Pharmacy in 2001and enjoys fossil hunting in the Dallas-Fort Worth area.

Panelist: Kelly Polinski, Senior Director, Population Health & Wellbeing at Brown & Brown

Kelly brings 14 years of experience in wellness, population health, and quality improvement.

She enjoys working with clients and their partners in strategic design with an overall goal to empower employers and their employees to achieve better health and care.

She is a former board member of the Worksite Wellness Council of Massachusetts where she also served as the Programs Committee Chair. In her previous role with the Cambridge Health Alliance, she was responsible for implementing and managing the wellness program for over 3,000 employees She also managed state-wide initiatives for the Healthcare Association of New York State (HANYS), including the HANYS Diabetes Campaign Collaborative. She is a Certified Wellness Program Manager through the Chapman Institute and is Certified in Public health (CPH). She earned her Master of Public Health from the University at Albany and her Bachelor’s degree in Biochemistry from Elizabethtown College. She has served as an examiner for Partners in Performance Excellence, using the Malcolm Baldridge excellence principles to strategically evaluate organizations.

She resides in Central Massachusetts with her husband and two children. She is a certified indoor cycling instructor and enjoys competing in sprint distance triathlons in her spare time.
